Reading a record
Facts with sources.
Unknowns left visible.
Profiles show available sources and last-checked dates. Fields nobody could confirm stay marked unknown. Call the program to confirm openings, intake hours, insurance and eligibility; the Directory does not recommend or refer.
How public records, allegations and locations are shown
- Where a public-record check has run, the profile lists what state licensing agencies, inspections and federal exclusion lists show, each with its source and date. “Public record: not yet checked” marks a research gap and says nothing either way about the program.
- Youth residential and inpatient programs show a license with its source and date, “No state license found in the records checked” with the date, or “License not yet checked.”
- A revoked, suspended or surrendered license, federal exclusion, substantiated abuse or neglect finding with a sanction, court-ordered closure, or closed program is taken out of default search. Its profile remains reachable by direct link with the official reason, date and source.
- A filed lawsuit or news investigation about patient safety appears in one fixed, dated form with its source and ends “This is an allegation, not a finding.” No program is removed on an allegation alone.
- Group homes, recovery residences, shelters and other small residences show city and phone only: no street address and no map pin.

The Directory’s search, sourced profiles, published program contacts and corrections stay free. Paying for Field never changes a program’s facts or order in results; claims and corrections do not affect placement. Teryli charges no referral or placement fees. Field is for professional outreach contacts, not client or patient information.
